You can call us during office hours at (606)268-3848 to speak with our staff. They will take your information and create your account. You can schedule your first appointment with them or use your account on our patient portal to schedule it yourself online.

We check for new submissions during office hours. Once reviewed, our staff will enter your information in our system. You will receive notification when your account is created through email. Once your account is set up, you can schedule your first appointment through our patient portal.

Please be advised that all in-person visits and telephone communications with Veterinary Care Center may be audio recorded for the purposes of accurate medical record documentation, quality assurance, and staff training. By proceeding with services, you acknowledge and consent to such recordings.
I hereby give Veterinary Care Center, including all veterinarians, employees and agents thereof, authorization to examine, prescribe, and treat my animal(s). I assume the responsibility for all charges incurred in the care of my animal(s). Iunderstand and acknowledge that payment is due at the time of service. I further understand, acknowledge, and agree that if I fail to make immediate payment, then I am liable for, and hereby agree to pay, all reasonable out-of-pocket costs and expenses (including, but not limited to, reasonable attorney and paralegal fees) incurred to collect for services rendered. Veterinary Care Center shall not be liable for any injuries or any damage to any person or animal, or be subject to any claim, demand, injury or damages, whatsoever, including without limitation, those damages from acts of passive or active negligence on the part of Veterinary Care Center its officers, employees, or agents. I hereby expressly forever release and discharge Veterinary Care Center from all such claims, demands, injuries, damages, actions or causes of action. I acknowledge that Ihave carefully read this paragraph and fully understand and agree that this is an unambiguous and unequivocal waiver andrelease of liability.

We proudly accept CareCredit and Scratch Pay in addition to cash, credit and debit cards.

Scratch Pay provides simple and transparent payment plans for veterinary expenses, without the need for a credit card, allowing you to choose a plan that fits your budget. Checking your eligibility for Scratch Pay does not impact your credit score.
